Before filling out a registration form, please check you are in our catchment area.

You will need to complete a registration form

You can do this by calling into the Surgery to pick up a registration pack or online by following the link below

Please note that if you are currently taking certain medications such as Diazepam or Pregabalin then we may not continue to prescribe these.  Also patients with long term conditions which require a sick note will need to be reviewed by a Doctor.

When you are completing your form please make sure you record any relevant medical history an current medication as we may not receive your medical records from your previous Doctor for some time and thus enables us to get to know whether you have any medical conditions which may need attention and also to make sure you receive any medication you require.

You will be asked for your height, weight and blood pressure on registering - we do have a biometric machine in our reception area which can be used
